At Locums United, we streamline the credentialing process to help physicians quickly and efficiently get started at healthcare facilities across the country.

How to Complete Your Credentialing Packet?

Follow these steps to ensure all your documents are processed smoothly:

Step 1: Collect Required Documents

Gather all the health, identification, certification, licensing, application, and background documents. Below is a comprehensive breakdown of the documents required, why they matter, and where you can obtain them. We’ve categorized everything for easy reference.

Physical Examination

Recent physical proving fitness for duty.

  • Where to Get It?

    Primary Care Provider or Occupational Health.

Drug Screen Attestation

Statement confirming drug screen clearance.

  • Where to Get It?

    Via AMN Healthcare or staffing team.

Immunization Records

Vaccination or antibody titers (for all listed below).

  • Hepatitis B

  • MMR (Measles, Mumps, Rubella)

  • Varicella (Chickenpox)

  • Where to Get It?

    Health Provider or Employee Health Portal.

Tuberculosis (TB) Test

  • Description:

    TB skin or blood test within 12 months

  • Where to Get It?

    Occupational Health or Urgent Care.

Vaccination

  • Description:

    • TDAP (Tetanus, Diphtheria & Pertussis vaccine (last 10 years).

    • Flu Vaccine (Current flu season documentation).

    • COVID-19 Vaccine (Proof of vaccine or exemption form).

  • Where to Get It?

    Provider portal or vaccination card.

Identification Documents

  • Driving License

  • Passport

  • Professional Photo

  • Social Security Card Copy

Background Check

Certifications & Licenses

  • Curriculum Vitae (CV)

  • Education, Internship & Residency Certificates

  • Service Letter

  • DEA Certification

  • Board Certification

  • State Medical License(s) Copy

  • BLS, ACLS and PALS (if applicable); American Heart Association preferred

  • Certificate of Insurance (COI)

  • Fluoroscopy Certificate (if applicable)

  • Case Logs

  • Face Sheet of Professional Liability Policy or Certification ($1/3 million minimum)

  • Malpractice insurance coverage (Past 05 years)

Step 2: Scan & Convert to PDF Format

All files must be digitized and saved as PDFs for standardization and secure upload.

Make sure the PDFs are clearly labeled (e.g., HepB_Titer.pdf, DEA_Certification.pdf, MMR_Record.pdf).

Step 3: Upload to LPU Portal System

After converting everything to PDF, submit your documents here.

After you submit the Phase 01 Credentialing Application, you can expect to receive a follow-up email from us regarding the next steps in the process.

What to Expect Next?

Your submitted documents will be securely stored in our credentialing portal system. Once we have reviewed your documentation, you will receive login access to your Locums United Credentialing Portal, where you can easily manage and upload additional documents.

Phase 02 Credentialing Application:

You will receive a Site Credentialing Packet requesting at least the following information:

CAQH & NPI Logins: Logins for the CAQH and NPI Registry – required to begin the credentialing process.

Pre-Application Form: The initial form required for site-specific credentialing.

MD Application: A comprehensive medical application form, which may include notarized elements.

Site Release Forms: Specific authorization forms required by the health system.

Payor Enrollment Packet: Documents necessary for insurance payor enrollment and billing setup.

⚠️ Please note: The Site Credentialing Packet must be completed and returned within 3 business days of receipt to avoid delays in your credentialing process.
